1. Acceptance of Terms
By accessing or using the website and services of Triumph Non-Emergency Medical Transportation ("Triumph," "we," "us," or "our"), you agree to be bound by these Terms and Conditions. If you do not agree with any part of these terms, please do not use our services.
2. Services Provided
Triumph provides non-emergency medical transportation (NEMT) services in Indiana and surrounding areas. Our services include ambulatory and wheelchair transport for patients traveling to and from medical appointments, dialysis, hospital discharges, facility transfers, and other non-emergency healthcare-related destinations. We do not provide emergency ambulance or paramedic services. In a medical emergency, call 911.
3. Booking & Scheduling
Rides may be booked by phone, email, or through our online form. We strive to confirm bookings within 1 business day. Same-day and urgent discharge transport is available subject to capacity. Recurring ride programs can be arranged for ongoing treatment schedules. We are not liable for delays or cancellations caused by circumstances beyond our control, including weather, traffic, or facility scheduling changes.
4. Pricing & Payment
Pricing is based on distance, vehicle type, service level, and other factors. We provide transparent, upfront pricing with no hidden fees. Payment may be arranged through private pay, or through Medicaid broker, managed care, or facility billing arrangements where applicable. Please contact us to confirm available billing options. Unless otherwise agreed in writing, payment is due as specified at the time of booking or per contracted terms.
5. Cancellations & No-Shows
Cancellations should be made as far in advance as possible. Cancellations made with less than 2 hours notice, or no-shows, may be subject to a fee. Facility partners may have specific cancellation terms defined in their service agreements. We communicate cancellations and changes promptly to all affected parties.
6. Patient Responsibilities
Patients and caregivers are responsible for providing accurate booking information, being ready at the scheduled pickup time, and informing us of any special requirements, mobility needs, or accompanying caregivers. Patients must follow driver instructions for safe transport and proper use of safety equipment.
7. Limitation of Liability
Triumph provides transportation services with reasonable care and in compliance with applicable laws. We are not liable for indirect, incidental, or consequential damages arising from transportation services. Our total liability for any claim is limited to the cost of the specific transportation service at issue. We do not guarantee specific clinical outcomes; our focus is on transportation reliability and safety.
8. HIPAA & Privacy
We handle all patient information in compliance with HIPAA and applicable privacy laws. Our Privacy Policy, available on our website, describes how we collect, use, and protect your information. By using our services, you consent to the information practices described in our Privacy Policy.
9. Insurance & Compliance
Triumph maintains general liability, auto liability, workers' compensation, and employer liability insurance. We are licensed and registered in accordance with Indiana NEMT regulations. A Certificate of Insurance and applicable licenses are available upon request.
10. Facility Partnership Agreements
Facilities entering into partnership agreements with Triumph may have additional terms defined in separate service agreements. In the event of a conflict between these Terms and Conditions and a signed service agreement, the service agreement shall govern for the matters it addresses.
11. Website Use
You agree to use our website lawfully and not to disrupt, interfere with, or misuse the site or its content. All content on this website, including text, graphics, logos, and images, is the property of Triumph or its licensors and is protected by copyright and trademark laws.
12. Changes to Terms
We may update these Terms and Conditions at any time. Changes will be posted on this page with an updated revision date. Continued use of our services after changes constitutes acceptance of the updated terms.
13. Governing Law
These Terms and Conditions are governed by the laws of the State of Indiana. Any disputes arising from these terms or our services shall be resolved in the appropriate courts of Indiana.
